How to order a test
Orders can be made on-line via our website, fax or by directly ringing Diagnostic Insight on (02) 9996 9991
Terms and Requirements:
• Patient must be referred by a Doctor or allied healthcare professional who is registered with Diagnostic Insight
Information required to make the order:
1) Patient Details:
• Patient’s name
• Date of birth
• Address to which the Kit is to be sent to
• Daytime Telephone Contact number
2) Doctor/Clinician Name required:
• Doctor/Clinician name and address
3) Payment details
A charge of $10 per Kit will be made for the Handling and Dispatch of the kit.
Payment method: Credit Card or Cheque/Money Order

4) Type of Kit/Test required
A test requisition form will be attached in the plastic sheath on top of the test kit. All instructions for collection of specimen and for returning the specimen to Diagnostic Insight are inside the Specimen Kit.
Payment details for the test itself must be written into the Payment Section of the Test Requisition Form before returning the specimen together with the form to Diagnostic Insight.
The charge for test will be made when the specimen is received by Diagnostic Insight. If required conditions for the specimen collection are not adhered to and the specimen is rejected for these reasons, a $25 charge will be made for processing the re- submission of the specimen.
Results are normally available between 2½ and 3 weeks from the time of receipt by Diagnostic Insight of the specimen.
Two copies of the results/reports (an Electronic Copy and a hard copy ) are then sent to the referring Doctor/Clinician only. Results are not sent to the patient direct as they are subject to interpretation.
